Parents are required to complete the medical form and affirm, on their honor, that they have fully and accurately disclosed their child's physical and psychological health to the school. This includes any history of hospitalizations, major medical treatments, psychological or psychiatric care, as well as behavioral concerns or legal issues. It is the responsibility of the parents to ensure that all relevant information is provided.
To the best of my knowledge, the information I have provided is accurate. I hereby grant permission for the child named above to participate in all camp activities, including but not limited to: canyoning, glacier expeditions, rock and ice climbing, water-skiing, tubing, wakeboarding, archery, sledging, torchlight walks, bridge swings, rappelling, karting, bowling, laser tag, paintball, dancing, ropes courses, swimming, mountain hiking, golf, indoor skydiving, paragliding, sailing, and general sports such as football, tennis, golf, and volleyball, unless otherwise noted in an attached medical report. I acknowledge the inherent risks associated with these activities and understand that they will be supervised by camp staff and carefully selected third-party professionals, all of whom meet the camp's strict safety and quality standards.
The camp is not liable for any loss of personal property, including money or valuables. Furthermore, the camp cannot be held accountable for additional costs, damages, accidents, delays, or inconvenience caused by third-party providers, except in cases of proven negligence by the camp itself.
I also grant permission for camp-selected medical personnel to provide treatment to the child if needed. In an emergency where I cannot be reached, I authorize the camp’s chosen physicians to administer necessary medical interventions, including hospitalization, X-rays, medications, anaesthesia, surgery, or other treatments unless otherwise noted.
